Christian Counseling Ministries (CCM) is a Christ-centered counseling ministry that offers Biblical counseling with compassion and grace.
We employ a holistic, results-oriented approach to help people heal and grow.
To schedule an appointment please call or email us, then print and fill out the Intake Form and the Permission for Exchange of Information (PEI) Form and bring them with you to your first appointment. If you do not bring the completed forms we will have to reschedule your appointment. The "Permission for Exchange of Information" is completely voluntary, however it must accompany the intake form even if it is left blank.
A hard copy with original signature is required. Please do not email your forms. All forms can be found below.
Please note: CCM is not an on-call counseling ministry, and as such, requires an appointment time. We are not able to accept walk-in or emergency situations. For emergency services, call 911 or other crisis hotline as appropriate.
